Russia’s Defense Ministry has confirmed earlier reports that the third stage of the Soyuz-2 launch vehicle was responsible for the failure to place the Meridian military communications satellite in orbit. The satellite subsequently fell to Earth in the Novosibirk Region, the ministry said. Judging from preliminary information, the launch vehicle failed “before the orbiting unit could separate from its third stage,” a source in the Russian aerospace industry told Itar-Tass. The Defense Ministry blames failure of the third stage’s engine for the incident.

    if it is a satellite then why is the 'cloud' moving with it and so steady as well as formed? why is it not that the 'smoke' can be seen coming off of the object as the object is traveling? it is questionable by the looks of it all.
